Hello doctor,

I am experiencing an erection problem where I cannot get hard anymore. Last month I had a motorcycle accident where I got an injury on the face and my foreskin, probably my penis hit the fuel tank. When hospitalized, the local doctor only treated the wounds on my face and let me get home on the same day by giving me painkillers and antibiotics. I could not differentiate the pain all over my body after that trauma. After getting home, I realized the tip of my foreskin was wounded too. It is like the skin was torn a bit, bleeding but not that much. When I went to the bathroom to urinate, little blood came out along with my urine. I wrapped a bandage around the wound and let it rest. The bleeding in the urine stopped in two days.

After a couple of days, my penis felt irritated sometimes and would always get stiff but not too hard. I avoided wearing underwear as it hurts sometimes. After weeks the wounds healed, but now I cannot get hard anymore. There is a dark thin vein-like underneath my shaft, which seems to be different than the other veins and arteries. I wanted to mention that there were internal bruises behind my right thigh. After three weeks after the accident, a small pocket of fluid formed inside. When touched at the top of the swelling area, I felt numbness. The fluid has started to disappear now, but the swelling and numbness are still there. It does not hurt that much. The pain is bearable when pressed. I am not sure if this is connected to the erection problem.

Can you help me, please? I am uncomfortable seeing a doctor too.

You had an injury near the penis and pelvis area, which can be the reason for your erection problem. Though you have mentioned that you do not want to see a doctor, I advise you to see a urologist because you may need a scrotal scan called a color doppler of the penis. This scan helps to find the blood supply of the penis and detect the cause of the problem. So for your situation, physical examination by a urologist and, if needed, penile color doppler is essential for diagnosis. Proper treatment can only be planned once the diagnosis and root cause of your erectile problem is detected. Most of the other injuries which you have mentioned will improve with time. Mild numbness in a few areas is common after swelling and inflammation of the area because of the involvement of small nerves in that particular area, but that is not very concerning in most cases. Bleeding in urine is a sign of urethral (urine tube) injury. It is also a self-limiting condition and subsides on its own, as it did in your case.

It is very difficult to predict now without knowing the injury status and physical examination. Usually, if scan reports and physical examination are normal, there are chances of recovery. However, you will need to be under the supervision of a urologist for some time, as they may advise you on medication for erection problems in proper dosage and schedule.
