Hi doctor,

I had foreplay the other night and stayed the night with the guy. Instead of going to the bathroom, I held it the entire night, and the next few days, I had a burning sensation when peeing, I tested for a UTI, but the results were negative. As urgent care, the doctor gave me an antibiotic shot yesterday. However, I woke up today and discovered visible blisters on my penis. What could this be? I have been having pain past three days. Currently, I am taking Descovy for pre-exposure prophylaxis. Kindly help.

I understand your concern. It looks like herpes genitalis (attachment removed to protect the patient’s identity), and it is a viral infection of a genital organ that is sexually transmitted. For diagnosis, I suggest you need a blood test for the Herpes virus and the rest of the investigations for STD (sexually transmitted disease) like a blood test for syphilis, hepatitis B and C, and HIV (human immunodeficiency virus).

Thank you for the reply. I had a herpes outbreak about three years ago, and I tested positive for herpes. But the initial outbreak was near my mouth, and this is new. Kindly help.

Yes, herpes can be near the mouth on lips, or it can be on genitalia. Herpes virus remains dormant, but at times it flares up. It causes pimples and pain, but it resolves by itself. Sometimes we use antiviral medications to make it dormant again. This virus cannot be cleared entirely from the blood. It remains in inactive status and gets activated. I suggest tablet Acyclovir 800 mg four times daily for 14 days.
