Hello doctor,

I am having some discomfort in my penis. It comes and goes but if I put pressure I do not feel anything. There is mild dull discomfort. When I sleep I do not feel anything but when I sit depending on if jean tightens that area it starts to pain.

Please advise.

I understand your concern. Please help me with a few details so that we can get to the bottom of your issue. Are you diabetic or hypertensive? Or have any other illnesses? Have you had any surgeries in the past? Do you have any difficulty in passing urine, such as increased frequency, straining to pass urine, urgency, or a feeling of incomplete emptying? Are you sexually active or masturbating regularly? Do you have any pain after ejaculation? Do you have any pain in the area between your testicles and anus? Have you undergone any tests in the recent past?

Thank you for the reply.

Not diabetic nor hypertensive. No illness. No surgeries. No difficulty passing urine, nor burning while urine. Nothing on the skin or anything different. No urgency that I can tell. No feeling incomplete. Yes, sexually active and I masturbate regularly. I had sex four days ago. No painful ejaculation. No pain between testicles. Have not undergone any tests. I did have long and rough sex seven days ago. I like to add I am a hypochondriac and after I think about something I start to see and feel things. The last time I had unprotected sex was about 21 days ago. I do not know if this is something important for you to know.

Thank you for reverting back with the details. My initial suspicion was whether you had some degree of prostatitis but with the details that you have given it seems unlikely. If you would like, we can run some basic tests just to make sure everything is okay. Or I could prescribe you some tablets for a couple of weeks to see if they make you feel better. They are not antibiotics but just anti-inflammatory and sort of a urinary cleanser. Which of the above two options would suit you more? Mind you, the availability of the medication depends on your current location and you might need a prescription from your local doctor as well.

My opinion is that this is just some urethral inflammation or urethritis if at all. Please run these tests so that we can rule everything out: Urine routine and microscopy, urine culture and sensitivity, and semen culture and sensitivity. These are the medications I suggest for a period of two weeks (you may need a prescription for them based on your location): capsule Chymoral Forte (Trypsin and Chymotrypsin), tablet Econorm (Saccharomyces boulardii ) 250 mg, and tablet Cranpac D (Cranberry extract and D Mannose). In addition to this, please drink at least 0.79 gallons of water per day and consume citrus fruit juice at least twice a week.

What fruit do you suggest? If I cannot take medicines, are there any home remedies or do you think this could be a serious thing that I will need prescription medications? Are there any over-the-counter medications or remedies?

I read your query and can understand your concern.

The medications prescribed are your best bet in my opinion. They are not antibiotics, they are just sort of supplements if I can put it that way. They help clean your urine up and alter the gut bacteria which has an effect on the prostate biome as well. I really cannot think of any home remedies other than maybe consuming lemon juice every day for a few weeks.
