Hello doctor,

My son is 37 years old. He had a swelling in the penis and a small marble-light bump in his scrotum. He was very sore on the penis and went to a urologist who recently diagnosed him as an epidemiologist. The doctor prescribed him Levaquin for ten days which he finished a few days ago, but he still had a sore and inflamed enlarged penis. The doctor put him on Celecoxib 200 mg. The swelling has gone down, and it has no muscle tissue feeling. He said when laying down that he can feel the blood flow. He is very uncomfortable. It does not seem to get better and seems to be getting worse. He is really upset and having a hard time emotionally. I need some kind of advice. Please help.

The description you gave indicates the diagnosis of dermatitis, and the best treatment is antibiotics covering gram-positive and anaerobes. So I prescribe tablet Augmentin (Amoxicillin) 625 mg three times daily and tablet Metronidazole (Flagyl) 400 mg three times daily both are for a week. He just finished Levaquin, and he is on the Celecoxib. Should he stop it later and then start with what you have suggested? What you are saying is this is an infection, correct and what causes this? Can I send you a photo in the morning? He finally went to sleep, so I do not want to wake him.

The Celecoxib (Celebrex) is not an antibiotic to continue for symptomatic treatment, and medications can be started at the same time tomorrow. The causes can be either due to sting from an insect or sometimes from infected hair follicles, especially if he got diabetes. The ultrasound is mostly useful for the scrotum or to detect abscess collection. If the swelling did not get worse, then I can wait. Yes, you can send me the picture tomorrow. I hope he will get better with the treatment.

It is a lot larger than it normally is. It is constantly painful, red, and swollen. I have trouble sleeping at night, and when urinating, it goes back, and then shortly after, it gets right back up to where it is now. I feel the blood flow every time I lay down, it rushes to that area, and it gets real swollen. It is just driving me crazy, and also, I just finished the other antibiotics. Levaquin and I are just wondering. Is it fine to continue with this one that you have prescribed? I did not receive any prescription. How do I go about getting them? How long does this type of condition last?
