Hi doctor,

I am a 27 years old male. I have two wisdom teeth in my lower jaw, which is impacted. And because of this, I suffer from TMJ pain. On my left side, I feel constant pressure or numbness with a formation of a small ball-like bulge seen slightly above the gums. I assume that “bumpy” looking gum is actually from the tooth as it erupts? The swelling has been present for the past 25 days, with no significant pain. On my right side, the impacted tooth has no symptoms but only a small pocket or hole in the corner that collects food. I was luckily able to remove any trapped food; therefore, there is no pain or swelling present. So here is my question, can I survive this, and for how long? I have an undiagnosed case of sleep apnea. I wake up with my hard pounding and the feeling of relief that I can breathe. Suffering from sleep apnea has 100 % convinced me that I would not survive wisdom tooth extraction. If I even survive these panic attacks, I am having. I hope the left-sided wisdom tooth erupts enough through the skin, so I do not have to pull out the tooth. At this moment, I refuse to get any treatment because I have fully convinced myself that this will be the end. Please help.

First of all, do not worry about the problem. You have TMJ (temperomandibular joint) problem, and the treatment is different for that. On the left side, you have pericoronitis, and on the right side, you have impacted teeth. Secondarily, you are taking medicine, which is not sufficient. So I suggest you please share if you have any drug allergy to prescribe the right medication for you. Regarding survival, you will survive it because treatment options have to be appropriately explored for good survival.

The Probable causes

Impacted teeth.

Investigations to be done

OPG (orthopantomogram).

Probable diagnosis

TMJ problems and pericoronits (to be confirmed after seeing picture and history).

Treatment plan

Take medications and wait.

Preventive measures

Warm saline rinse every 12 hour for 1 week.

Regarding follow up

If pain persists.
