Hi doctor,

I am suffering from dark colored dry patches around the mouth, which are swollen and very painful. I am only on some cream for acne. Please recommend some medicine for dry patches.

Your history and clinical images (attachment removed to protect patient identity) are suggestive of a perioral variant of acne. Also, long term use of certain creams for acne can make your skin susceptible to such patches at the angles of your lips. Kindly mention the duration of the lesions, any preceding pus filled lesions at these sites and the names of the creams you have been using.

Differential diagnosis

1. Perioral dermatitis.

2. Gram negative folliculitis.

3. Perioral acne.

Treatment plan

1. Capsule Doxycycline 100 mg twice daily after food for two weeks. 2. Peroduo gel (combination of Adapalene and Benzoyl peroxide) to be applied in a peanut volume, daily in the evening for 3 to 4 hours duration all over the face. In case of dryness or irritation, use it alternate days. 3. Use sunscreen in the morning.

Thank you for the reply.

I have never used any medication around this area that is, around my lips. I have already used Peroduo gel on my cheeks for acne. I also got these types of patches on my hands. It has a few cuts too, which is very painful and I have faced them since the start of winters as this is due to dry skin.

If you have already been using Peroduo, then all your complaints can be explained as retinoid dermatitis (a common side effect of topical retinoid therapy). Peroduo is known to cause dryness of both skin and lips after regular use, especially without breaks and for long hours of application. I would advise you to stop using it temporarily in such a case. It would be preferable to use good sunscreen in the mornings and non-greasy moisturizers in the evenings along with an antiacne face wash for two weeks to let the effect of retinoids fade off.

Treatment plan

1. Use sunscreen in the morning and face wash in the evening. 2. Clindac A gel (Clindamycin) for the affected areas in the evening. 3. NMF E cream at night after face wash (Aloe vera and Vitamin E). 4. Continue Doxycycline for two weeks.
