Hello doctor,

I am a 21 year old female, and I have seen a lot while growing up. Since I was 6 years old, I started crying every day. Fear grew within me. I started having bad grades since I was 11. I was bullied twice by my friends when I was 7 and second time when I was 13. My parents had no time to love me and indulged in verbal abuse all the time. It made me feel even more depressed till today. I find pleasure in other ways that keep me alive. It is just a living and not a life. I am so very tired deep down. I have no one to talk to wholeheartedly. I feel lost and all by myself. I just want some affection and a person who understands me and lives through me. I feel incomplete. I have not consulted any therapist or not under any medication. Thank you. Please provide me with some peace.

First of all, I would like to tell you that your problems are temporary and have a cure. Poor parenting and adolescent non-supportive environment may lead to above features. Our personality development is mainly affected by environment and past experiences. But you are only 21 years old. Low mood, emptiness, loss of interest, low energy, and low self-esteem, are all symptoms of depression. In this situation, I will suggest you visit a nearby psychiatrist. Few antidepressants like Escitalopram and Desvenlafaxine will help you in early recovery from depression. My mom was diagnosed as mentally sick eight years back. She is found having triple behavior. One is normalcy, other is a child personality and finally an aggressive person. She changes them unexpectedly. She is under medication. But still, there is no change except for a minimum control. She assumes a lot and overthinks terribly. She becomes very restless and loses her sleep. Before eight years, that is before she was identified mentally sick, my father and mother used to have severe problems due to her overthinking and unusual doubts. Then suddenly it blew up eight years back, and she became like this. Her current symptoms are, she assumes my dad and me as a god, and she expects things to happen as miracles, but she knows I am her child and he is her husband. But otherwise, she does this. She assumes that my deceased grandfather is alive and stays in our city somewhere, and she asked this to my cousins, which shocked my family. She assumes that my friends do bad to us and that I have a twin sister. She thinks that my aunt took away my twin sister from her. She takes Sulpitac 50 and Pacitane. Please let me know if these are the right medicines doctor. Please suggest as to what has to be done. Thank you so much.

Thanks for this query. I can understand your concern regarding your mother. You have given an elaborated history which is clearly explaining that your mother is having suspiciousness, delusions, hallucinations. It shows that she has a delusional disorder like schizophrenia. According to you, she is taking Sulpitac 50 (Amisulpride) and Pacitane (Trihexyphenidyl). I think 50 mg is not an effective dose. She will need minimum 200 mg Sulpitac for complete relief. 100 mg Sulpitac in morning and night. If the symptoms still persist after these changes, combination therapy with 10 mg Olanzapine will be required. Pacitane should be given in same doses. Psychiatry medicines should be given in optimum doses. A lower dose will not block all the receptors and symptoms will not be cured. You can talk with her doctor about the doses as I suggested. Keep in mind that the doses should be increased gradually, the first week total 100 mg Sulpitac, then 150 mg second week, then 200 mg on the third week and continue it for minimum one year. It will reduce the chances of relapse and she will be able to live disease free healthy life. Hope my answer helped you. You can ask anytime if you have any doubt. Regards.

Such type of delusional disorders are results of overvalued ideas, which become delusions under severe stress and anxiety. Severe stress for a long time without emotional support in the family may lead to this type of a behavior. Good mental health is very important, and as you mentioned her past situations, she was under an enormous stress and anxiety. Slowly it resulted in suspiciousness, which progressed in firm belief. But do not worry, it is curable, after optimum doses as I suggested, she will be completely fine. Have patience, these medicines will take almost a month for complete recovery.
