Hello doctor,

My 9-years-old son has gotten this rash (pictures attached) on the cheeks and ear for the last couple of weeks, especially after a long night's sleep. They disappeared after one or two days, and he got them again. It appears only on cheeks and ears, not anywhere else. He had got these rashes two years before. He does not have a fever or any other infection. His heads get sweaty at night for the first two to three hours. Are this rash petechiae? Is this something serious?

As per the information provided and looking at the pictures (attachment removed to protect the patient's identity), is there a possibility of an allergic reaction or an insect bite? Before going further, I would like to know a little more information. Is he complaining of itch? Is it painful to touch? Have you noticed any swelling in his neck? Does he uses any cream or lotion to apply at night or washes his face with soap or gel? Is anyone else in the family get similar rashes on any body parts? Does he sleep in his room separately on a single bed? I would suggest you the following medicines:

  1. Syrup Claritin (Loratidine) 5 ml once daily for three days.
  2. Calamine lotion for local application over the lesions. Consult your specialist doctor, discuss with them and take medicines with their consent.

At this stage, I would not advise you to go for any investigation. We can decide that only if the problem persists or after getting other positive information,

Hello doctor,

He does not complain of pain or discomfort, and I did not notice anything unusual on his neck, no change in shampoo and lotion. I started using Aquaphor on his face three months ago and began having rashes two weeks ago. It is not an insect or bug bite. No one in the family got there, just him. I change pillow covers and bed sheets every alternate day. It seems petechiae to me as per its description seen online. What kind of investigation would be needed for this? If this is petechiae, then what could be the possible causes?

As it only appears on his face that too mainly at night, and he sweats before that, it is a bit unusual to be related to petechiae, but still, we need to confirm with some blood work. I am suggesting some investigations below. You can get it done directly or show it to your family physician, such as complete blood count (CBC), C-reactive protein (CRP), estimated sedimentation rate (ESR), peripheral smear for platelet morphology, prothrombin time (PT), activated partial thromboplastin time (APTT), and liver function test (LFT). These are the basic investigations, let me know the report, and we can plan for further action. Start the medication within three days.
