Hi doctor,
I have dark pigmentation in various areas of my body, including my underarms, neck, elbows, and knees. Some of these areas also have an unpleasant smell.

I understand how distressing it can feel, especially when pigmentation affects visible and sensitive areas like the neck, underarms, and body folds.
Darkening in these regions is quite common and can occur due to several factors, such as friction, frequent shaving, a buildup of dead skin, hormonal or metabolic conditions like insulin resistance, excessive sweating, or recurrent mild infections that can also lead to odor, particularly in the underarms.
To care for the affected areas, clean them with a mild, non-fragranced cleanser and keep body folds dry to reduce moisture and odor.
Wearing loose, breathable cotton clothing and avoiding harsh deodorants or scrubs can help prevent further irritation and darkening.
For treatment, consider using a 6% Glycolic acid cream. Apply a thin layer to the dark areas on alternate nights.
Glycolic acid works by exfoliating dead skin cells, improving texture, reducing pigmentation, and controlling odor-causing bacteria over time. Avoid applying it immediately after shaving to prevent irritation.
Keep in mind that results take time, and visible improvement is usually seen over several weeks. If the pigmentation is severe or spreading, it is advisable to get your fasting sugar checked to rule out conditions like acanthosis nigricans.
You may also share a picture of any affected area in good light so that I can provide more specific guidance.
With consistent care and patience, the skin tone and comfort of these areas can improve significantly within four to eight weeks.
Additionally, if you are overweight, try to manage or lose weight, as obesity can worsen pigmentation due to friction in folds and conditions like acanthosis nigricans.
