Hello doctor,

My mother-in-law was operated on fourth-degree piles two years back. Nowadays, she has been bleeding for the last 15 days. Sometimes before passing stools. It gives her abdominal pain. She had bleeding when a colonoscopy was conducted to confirm cancer. As per the specialist, it was not cancer. Now again, bleeding started, and she cannot undergo colonoscopy in this old age. Kindly advice.

As you mentioned, she had surgery for piles, and now she again has developed bleeding, so I would keep the possibility of recurrence of piles. This can only be confirmed after the doctor physically examines her, so I would advise you to visit a doctor to examine her and make the correct diagnosis. Secondly, you can start her on the following treatment for the time being. This will give her relief. I suggest the following:

1) Tablet Tranexa (Tranexamic acid and Mefenamic acid) 500 mg thrice daily for three days.

2) Tablet Daflon (90% Diosmin and 10% Hesperidin) 1000 mg 2 tablets once a day for seven days, then one tablet once a day for two weeks.

3) Syrup Looz (Lactulose) 15 ml twice daily.

4) Anovate (Phenylephrine, Beclometasone, and Lidocaine) ointment thrice daily.

However, this is only a temporary treatment. You will have to visit the doctor for permanent treatment and get some tests done.
