Hi doctor,

I am a 21-year-old male. I observe symptoms like small white boils or pimples on the head of the penis and also difficulty in pulling back the foreskin. These symptoms started four days ago. I am not on any medication and have no previous history of the same. Kindly help.

Kindly answer me the following to help you better. Do you feel soreness in the lesion? Is there a pearly papule appearance? Is it an inflammatory lesion? Is there any redness? Do you have swelling of the glans? Excess scratching and rubbing of lesions? Is the lesion itching? Is it recurrent balanitis? Kindly attach the clinical picture of the lesion to assess and arrive at a definitive diagnosis and guide you accordingly. Thanks.

Thanks for the reply. There is no swelling or redness. But it is itchy. Yesterday, I observed an accumulation of white paste-like things on my glans. Kindly help.

It is balanitis with pearly penile papules and folliculitis. I suggest you apply Candid cream (Clotrimazole) twice daily at noon and night for 15 days and Fucidic cream (Fusidic acid) at day time for ten days. And take tablet Levocetirizine 5 mg at night for seven days. Kindly clean your genitals with a Cetaphil cleanser (Propylene glycol, Methylparaben, Propylparaben) daily. Kindly avoid friction, occlusion, and moisture. And also, keep the area dry and clean. Avoid scratching and rubbing of lesions as it may cause secondary bacterial infection. Maintain proper genital hygiene. Always retract the glans and clean them with lukewarm water. Follow up after seven days. Regards.

Thanks for the reply. As Fucidin cream is meant for external use, is it safe for the glans area? I am also sending another image for confirmation. Would you please assure me that it is not STI or STD? Thanks.

Fucidin cream can cause minor irritation, but it is fine. It is advisable to get tested for all STD (sexually transmitted diseases), urine routine, urine culture and sensitivity test, microscopic tests, VDRL (venereal disease research laboratory), HCV (hepatitis C virus), and herpes 1 and 2 tests. I suggest you take tablet Ciplox 500 mg (Ciprofloxacin) once daily for three days and tablet Zifi 200 mg (Cefixime) twice daily after food for five days. Kindly maintain proper genital hygiene, practice safe sex, and avoid multiple partners. Get all tests done and follow up with reports. Regards.
