Hi doctor,

I have had something on my back for the past one year. I guess it to be pityriasis versicolor, but I am not sure about that. I do not take any medications. Kindly help.

It is a fungal infection with an atopic rash. I suggest applying Candid lotion (Clotrimazole) or Kevon lotion (Ketoconazole) 20 minutes before bath for 20 days. Use Dersil-AF body wash (Ketoconazole) or KZ soap (Ketoconazole) for bathing for one month. Use Candid dusting powder (Clotrimazole) to control sweating and take tablet Loratadine 10 mg once daily before breakfast for ten days. Do you have a fever, itching, irritation, burning sensation, excess sweating, hyperpigmentation on sun exposure, spreading margins, dry scaly appearance, any previous history of atopy or hives? Kindly answer the above questions. Regards.

Malassezia fungus infection (pityriasis versicolor) is the diagnosis. The predisposing factors are a warm or humid environment, sweating, and improper hygiene. I would suggest topical treatment along with the treatment already mentioned. Apply Miconazole 2 % ointment twice daily for two weeks, maintain proper hygiene, wear loose, comfortable thin cotton clothes, avoid scratching or rubbing the lesions, and keep the area dry.
