Hello doctor,

I am 34 years old (weight 171 pounds), married, and my height is 5 feet 11 inch. I feel lack of libido during sex. During sex, I do not feel excited enough and do not get erection needed for sex. I use my hand and erotic thoughts to get the erection first and complete the task somehow as we are planning for a child now. However, after four to five days of daily sex, even this process fails and my desire is lost. I take Revital-H daily. I do not take or have taken any other medicine for this problem. Kindly advice.

The causes of these symptoms are multiple including medical conditions as well as psychological in nature. So, we first need to rule out the medical conditions that may be responsible for these symptoms by some investigation such as blood sugar, thyroid function test, serum testosterone, prolactin level and routine blood count as a minimum investigation and also get the semen analysis done. For this, you need to consult a psychiatrist or urologist in person so that he can examine you and advise necessary investigations that may be required. If there is any abnormality detected in these investigations then management includes treating the underlying cause. If everything comes within normal range, then a drug like Bupropion and PDE (phosphodiesterase) inhibitors like Sildenafil or Tadalafil are helpful to manage libido as well as erectile dysfunction. Multivitamin supplements are also helpful to some extent. Consult your specialist doctor, discuss with him or her and start taking the medicines with their consent. Also, spend some good time with partner and spend enough time during foreplay. For this, you can take a vacation during a weekend and visit a place you had enjoyed earlier. Avoid any use of alcohol, tobacco or any other substances, and if taking currently then stop. Do regular exercise and get adequate sleep. I hope this helps. Thank you.
