Hello doctor,

I have difficulty getting and maintaining an erection. My erection is weak, and I cannot penetrate. When I try to penetrate, I lose my erection, and it lies on the left side when erect. My penis is soft and loose and of small size like a kids penis. My midnight erection and early morning erection is average, but I lose erection when I am fully awake. Achieving and maintaining a day time erection is very difficult, and I cannot penetrate.

Your reports tell that you are having hyperprolactinemia (attachment removed to protect patient identity). You need to consult an endocrinologist for that. This condition can cause erectile dysfunction. However, your Doppler study of the penis is within normal limits. Tadalafil will work best. But it needs neurogenic stimuli for initiation of action, as Tadalafil works on blood vessels and dilates it. Hence it causes an erection. But for its actions to happen, you need to have a neurogenic stimulus i.e., foreplay. It is a prerequisite for the erection to occur. So when you are planning for sexual intercourse, please take Tadalafil 30 minutes before the possible time of action and do involve in foreplay for an optimum effect of Tadalafil. And you need to correct your prolactin levels, and few more investigations may be needed for that. Your endocrinologist will tell you details regarding this.
