Patient's Query

Hi doctor,

I tested COVID-19 positive this month. I have attached my old prescription and the latest prescription with the test results. Even after 25 days, I still have a fever daily with 99.4, 99.5, 99.6, 100 degrees Fahrenheit. Today morning it was 99.4 degrees, and evening 100 degrees Fahrenheit. Also, when I lie down to sleep every alternate day, one side of my nose gets blocked. I have to use Karvol Plus or Vicks inhaler and Pulmoclear tablet. For fever, I am trying Paracetamol, Dolo and now started Mefenamic Acid 500 mg. Both eyes have allergic conjunctivitis. I have attached the pictures. Even after 25 days, I do not see any control on fever or nose, which is getting blocked. I have little phlegm or mucus and have to gargle it out. A week back, I had sinusitis-like pain, and my head felt heavy. It feels like my sinus is draining, but I do not have any running nose, sneeze, and cough. My HRCT was 0 after four days of testing positive. I have attached the reports. My CRP is 2.3, and my D-dimer is 239 ten days back. Also, my RT-PCR came out to be negative as well. Please review and suggest the best course of action.

Thank you for the query. I can understand your concern. According to your statement, you have been suffering from fever for an extended period. You have also been suffering from a headache. You have little phlegm or mucus. First of all, in medical terms, any temperature of 100.4°F or above 100.4°F is considered as fever. Raised body temperature may result from infections like viral, bacterial, or fungal, medications, dehydration, blood clots, etc. Your current use of medications such as steroids like Dexamethasone may cause raised body temperature. But, again, dehydration may also cause increased temperature.

Your redness of the eye may result from allergies. In the case of conjunctivitis, the white portion of your eye may become more red or pink, swelling of the conjunctiva, tear production will increase, itching, irritation or burning sensation, urge to rub the eyes, etc. It may be the initial stage of developing conjunctivitis, but conjunctivitis has not been developed yet.

You can undergo some investigations like the Widal test, Weil-Felix test, urine R/M/E (routine microscopy and examination), triple antigen test, HRCT (high resolution computed tomography) of the chest, serum creatinine, etc., to exclude the possible causes for fever. In the meantime, you can take Cefixime 400 mg twice daily along with Azithromycin 500 mg once daily for both typical and atypical bacterial coverage. In addition, you can apply antihistamines eye drops along with antibiotics eye drops in your affected eye. Take plenty of fluids to keep yourself hydrated.

Thanks for the query. If you are still suffering from fever, then you can undergo the following investigations,

  1. Widal test.
  2. Weil-Felix test.
  3. Urine R/M/E.
  4. HRCT of the chest.
  5. Triple antigen test.
  6. Serum creatinine.
