Hi doctor,

I am extremely worried. I was rushed to a hospital two days ago to find out that I was pregnant for ten weeks and four days. I got the Depo shot eight weeks ago and was unaware that I was pregnant. I have been bleeding for two and a half months straight (filling up my pad every 15 to 30 minutes), and in the hospital, they said everything was good and that the baby’s heart was beating well, and I was moving around a lot. I have been in extreme pain today and passing massive blood clots of the size of golf balls during the last five times when I was urinating. I am scared that I am miscarrying, but can that be possible if the ultrasound showed the baby was fine two days ago? I need some reassurance even as to what it might be.

Two days back, you had a scan that showed a healthy, alive baby, but after that, you passed big clots along with blood.

1. Better to go to the hospital and get a rescan now to recheck the current situation.

2. When did you have the Depo (Medroxyprogesterone acetate) injection? The side effect includes unscheduled bleeding, spotting, mood variations, and breast tenderness. Withdrawal bleeds also could be a side effect of this.

3. But the passage of big clots needs rechecking by scan again. It could be a threatened miscarriage, or you might need urgent intervention, as too much bleeding will cause weakness, hypotension, and dizziness. Generally, the body can be affected by heavy blood loss.

You need to do a complete blood count and blood grouping with cross-matching and save serum in case you need a blood transfusion.

If pregnancy remains stable and bleeding subsides with normal scan findings, you need Folic Acid, iron supplements, and some progesterone support to help in pregnancy stability.
