Hi doctor,

For the past eight years, I have been living with PCOS. My last period was on four months back, when I withdrew from Medroxyprogesterone. For the following three months, I did not have any periods and refrained from sexual activity. This month, I engaged in sexual activity while taking protection. I consulted my doctor and she prescribed me Medroxyprogesterone for seven days. I started having cramps and on the 11th day after starting the pill I had very faint bleeding. On the first day of my period, I had normal blood clots and red cervical mucus during urination as usual. However, now on the third day, I am not getting any period flow and the red mucus has also reduced. I am scared and wondering if this could be a pregnancy. Additionally, I started taking oral contraceptives as prescribed by my doctor. I am confused as to what is going on in my body and if all these things might result in a pregnancy. What should I do? I would be grateful if you could help me. Thank you so much.

If you have had polycystic ovary syndrome (PCOS) for eight years and have been having withdrawal bleeds with hormone treatment, but are now unable to have regular bleeds, even with hormone treatment, it would be beneficial if you would send me your detailed gynecological, obstetric, medical, and surgical history. It is important to be on the safe side and have a serum beta HCG (blood pregnancy test) done as soon as possible to determine your pregnancy status. If the result is negative, I suggest taking tablet Norethisterone 10 mg daily, twice from day four of your periods until day 26 and then again from day four till day 26 for three cycles. I recommended having the following investigations on the day second of your period:

  1. Serum prolactin.
  2. Testosterone.
  3. Insulin.
  4. Progesterone.
  5. FSH (Follicle - stimulating hormone).
  6. LH (luteinizing hormone).
  7. Thyroid function test.
  8. Transvaginal scan (ultrasound).

Please do the above investigations and revert with the reports here. Here I suggest some tips which may help you to get rid of this. Maintain a healthy lifestyle, such as having a daily walk, exercise, and yoga to help keep a normal BMI (body mass index) and weight. Have a balanced diet with less sugar, salt, spices, and oil as well as ensure plenty of fruits and vegetables are being consumed and have recreational activities such as swimming, reading, dancing and avoid recreational drugs.
