Hi doctor,

My wife is 24 years, and she had a regular menstrual cycle even after she was on Triquilar tablet for six months after marriage. This month she observed only spotting but no bleeding. I tested at home for hCG urine, but the result is negative. Should I retest or continue Triquilar as I cannot visit a gynecologist in this pandemic? What should be my next step? She also has a slight stomach ache and nausea since ten days. Please guide me.

I would recommend you repeat another home pregnancy test (from a different brand) with an early morning first sample of urine as it contains the highest levels of hCG (human chorionic gonadotropin). However, if even this is negative, you may wait for the menstrual bleeding to start fully and continue to take the pills without any worry. Also, if Triquilar (Levonorgestrel and Ethinyl estradiol) is not being taken from day two of your cycle, kindly use backup contraception to prevent pregnancy. Also, if your wife is on any other medication other than pills, it can reduce the efficacy of the pill and cause changes in the menstrual cycle. I hope this was helpful.

As we cannot visit a gynecologist in person right now because of the lockdown, my wife is enclosing an image here for your reference. Please see the image and let us know how to proceed next. As I am a doctor myself, I was reading about cervical plug in early pregnancy. She observed white discharge yesterday that stopped later on. We have discontinued Triquilar, and I need to check with another home pregnancy kit. After how many days should we check again, and if it is negative, should we go for an hCG blood test? Kindly give your opinion.

I have looked at the picture attached (attachments removed to protect the patient's identity) in that I could not visualize a cervix or a mucus plug (needs speculum examination, only the anterior vaginal wall is visible). Also, the mucus plug is thicker and more solid than the discharge visible in the picture. Physiological white discharge is increased just before menstruation also. So increased white discharge would not be a very confirmatory sign of presence or absence of pregnancy. Urine pregnancy tests are most effective and accurate 35-40 days after the previous menstrual cycle date. Hence kindly check during this time period. Also, if this turns to be negative, I would suggest an abdominopelvic scan as it would help to either confirm the presence of pregnancy and also help diagnose the cause of delayed period or reduced bleeding in the absence of pregnancy.
