Hi doctor,

I consulted you before for my shortness of breath. I was having a lot of PVCs and still am. I am curious about how frequent they are. And should I be evaluated more if I need medication? My blood pressure is really low, like 90/55. I am sending you some EKG’s that I got with my portable monitor and some from my fingers, and some by attaching one electrode to the heart and the other to the fingers. Are my PVCs too frequent? I have done an ultrasound, heart MRI, and stress test five months back, and it did not show any abnormality, whereas some of my ECG readings had arrhythmias and ectopic beats. Please help.

In the attached tracings (attachment removed to protect patient identity), I could not see any PVCs (premature ventricular contraction) in the recent tracings. Some tracings show movement artifacts (which is read as unreadable by the machine), and some show low voltage complexes, where occasional PVCs might have been missed. But otherwise, no evident PVCs are seen. Did you have symptoms while tracings were being obtained? Now the tests which you underwent are enough, and further tests are not necessary unless there are new symptoms or PVCs becomes very frequent like more than 10 percent of all beats (this can be known on Holter, very unlikely in you considering your recent tracings) or occurring in a row without normal beats (three or more). Treatment is necessary if we find any abnormalities on tests or too symptomatic to bear. Regarding blood pressure, your weight is also low. So if you do not have any symptoms like dizziness on exertion or too much fatigue and your BP has always been like this, then this probably is an appropriate weight BP. I hope this helps.

My Kardia (manual monitor) says the little dots right under the waves are my heartbeats. When I look, I see the space between the dots are too frequent sometimes, which I assumed were premature beats. While noticing the unreadable reading, I see a lot of dots coming right after each other. So they are not PVCs? My symptom is shortness of breath.

No, those are not PVCs. Dots do indicate the heartbeat, but that can be misleading at times. Machine mentioned it unreadable because movement artifacts lead to irregularities in the tracings, and for the same reason, you got extra dots underneath. After seeing the complexes, one can conclude that those are normal beat only. So your shortness of breath is not related to the heart, and other factors like deconditioning, low hemoglobin should be sought.

Same symptoms don't mean you have the same problem. Consult a doctor now!
