Hello doctor,

I am 26 years old, and my period cycle lasts for 35 to 40 days. I got my periods after 46 days of the last periods. I had heavy bleeding for three days and normal bleeding, which lasted for one week, but this time I had a light period for the first four days and then normal bleeding for 12 days. I did not have any back pain or anything, and no blood clots also. Also, the bleeding has not stopped even after 12 days. Kindly help.

Some hormonal imbalances, local pathology, and externally induced hormones can cause irregular bleeding. Therefore, if you still have bleeding at present, I would suggest the following:

  1. Tablet Ponstan (mefenamic acid) 500 mg and tablet Tranexamic acid 500 mg, both eight hourly for two to five days if symptoms continue. Stop the medicine once the symptoms stop.
  2. Have complete blood count, thyroid function test, cervical smear (to exclude cervical pathology), transvaginal scan, and local hysteroscopic examination to exclude endometrial and cervical polyp and any other local causes.
  3. Have tablet Norethisterone 10 mg twice daily from day five to day 24 for three cycles to regulate cycles and to prevent unscheduled bleeding.
