Hello doctor,

I am a 37-year-old male. Three months ago, a pain started in my right side testicle area, scrotum, groin, and right side upper thigh near the scrotum, after masturbation. There is no fever associated. I took the prescribed tablet Levofloxacin 250 mg and tablet Nimesulide 100 mg for ten days, but the pain did not decrease. So, I was given tablet Levofloxacin 500 mg and a combination tablet of Diclofenac and Paracetamol for ten days. I also started having frequent urination and an urge to pee every hour. I then took another antibiotics course of tablet Ciprofloxacin 250 mg for ten days. However, I still have some discomfort. Similar pain had occurred a few years before that resolved within a month with medications. In addition, there is a mass-like feel below the left testicle when I self-examine, and it also shows in the scrotal ultrasound. What causes a bump below the left testicle?

I understand your concern. Your sonography report (attachment removed to protect the patient’s identity) shows mild swelling in the left epididymis (starting part of sperm tube). There may also be swelling in the prostate, which would explain right scrotal pain. I suggest you to follow a few instructions, get a urine culture test done to change the antibiotics depending on the culture report. Continue tablet Levofloxacin (antibiotics) 500 mg once daily for 15 days. Kindly consult a specialist, talk to them, and take medications with their consent. Avoid sitting in one place for a prolonged duration. Avoid cycling for two to three months. Follow up if the symptoms do not resolve.

Thank you for your reply.

I have been taking antibiotics for the last three months. I was also diagnosed with COVID-19 a few months before, so I took antiviral medications, tablet Doxycycline 100 mg and tablet Paracetamol 650 mg, for almost a month during that time. I am concerned that my kidney, liver, and bladder may be affected due to taking so many medications continuously. What should I do?

To treat prostatitis, oral antibiotics must be taken for one to one and a half months. You have taken 30 days of antibiotics, but it included ten days of tablet Levofloxacin (antibiotic) inadequate dose (250 mg). I suggest you follow the instructions given previously. Prostatitis can have a recurrence, but subsequent episodes are of lesser duration and milder intensity.
