Hello doctor,

My sister is 27 years old. She has normal IOPs, right eye 11 and left eye 13. She has had problems with focusing while going down in the stairs. She went for an eye test, and her fields are attached. Our maternal grandmother has open angle glaucoma. There is no other relevant family history. She suffers from Sjogren's and visits an ophthalmologist regularly. She has large deep symmetrical discs. Do you think she needs urgent attention?

The visual field is suggestive of congruous homonymous inferior quadrantanopia (pie on the floor), visual field defect in a limited quadrant. This is indicative of neurological problem most likely in the parietal lobe of the brain. I would advise MRI brain and urgent medical attention. Take care.
