Hello doctor,

Recently, I noticed a rash on the meatus, which looks reddish after ejaculation. I have tested negative for all sexually transmitted diseases except for Mycoplasma genitalium (Mgen). I have also noticed that my tongue has a white film that keeps reappearing even after I brush.

Please help.

I went through your query and understood your concern. Please give me a few more details about your overall health. Are you diabetic, hypertensive, or have thyroid problems? Have you recently taken a course of antibiotics? Do you smoke? If so, how many cigarettes per day? For how long have you noticed the redness on the meatus? Have you had any unprotected sexual activity recently? Do you have difficulty passing urine, like - straining, urgency, a feeling of incomplete bladder emptying, or a burning sensation? Has there been any discharge from your meatus? I cannot see anything wrong (regarding the meatus) in the attached photo (attachment removed to protect the patient's identity). Please revert with further information.

Hello doctor,

Regarding your queries, I am not a diabetic, hypertensive, or thyroid patient. I have not taken any antibiotics recently, but I smoke all day. I noticed red meatus three weeks ago and had unprotected sex several times over the past few months. I have more than normal dribble at the end of urination but no discharge from the meatus. I have attached another picture for your reference.

You may have developed urethritis due to unprotected sexual contact (especially with a new partner). Therefore, I suggest you take the following medications; after consulting your local doctor, discuss with them and take the medicines with their consent. Tablet Chymoral Forte (Trypsin-Chymotrypsin) three times a day for one month, tablet Cranpac D (Cranberry extract and D-Mannose) three times a day for one month, capsule Econorm (Saccharomyces boulardii) 250 mg twice daily for one month, and capsule Urimax (Tamsulosin) 0.4 mg at bedtime for one month. These medications may cause retrograde ejaculation, which will settle once you stop using the drug.

I also advise you to get urine routine and microscopy, urine culture and sensitivity, and semen culture and sensitivity tests done. Based on the above test results, I will prescribe antibiotics. In addition, I would advise you not to participate in any sexual activity for the next month and use a condom if you participate in sex.
