Hi doctor,

I have red spots on my lower leg, and they are itchy. I have had these spots for months, and they keep coming back. I am not sure if it is bed bugs or something serious. Please advise.

I suggest you apply Fucibet (Betamethasone and Fusidic acid) cream at night for ten days. Please answer the following questions for a better understanding of your problem:

I am not diabetic. Yes, it itches at night time. What is it?

I understand your concern.

It is an allergic reaction to itch mites (hypersensitive reaction to itching mite antigen-scabies as night aggravation of itching occurs in scabies). Following are my suggestions for your problem:

  1. Apply 5 % Permethrin cream at night on day one and day seven. Apply below the neck and all over the body, including lower limbs, upper limbs, chest, and back -region. The contact period of the cream should be 8 to 12 hours. Repeat similarly in another week on day one and day seven.
  2. Apply Fucibet cream at night for ten days.
  3. Permite BB bar soap (Elestab) or Cetrimide soap for the bath for one month.
  4. Tablet Teczine-M (Levocetirizine and Montelukast) once at night after food for seven days.
  5. Tablet Ivermectin 12 mg once a week for two weeks.
  6. Apply Mupirocin cream twice daily for ten days.
  7. Avoid scratching and rubbing of lesions.
  8. Maintain proper hygiene.
  9. Avoid contact with pets, dust, pollutants, and allergens.
  10. Wash clothes separately in hot water. Dry them inside out in the sun. Iron them.
  11. Avoid sharing personal belongings as it is contagious.
  12. Treatment is similar for others if anyone in the family has similar complaints.

Follow up after two weeks.
