Hello doctor,

I take Emsam, the transdermal patch. I began on 6 mg around six months ago and everything was fine. The drug worked and no skin irritation or itching. Three months later I upped to 9 and then 12 mg and my skin began to have visible reddening and bruising at application sites. It also began itching after 10 or so hours after patch placement. Now at 12 mg it begins to really itch bad within 2-3 hours. All the topical ointments/cream have been tried: rubbing alcohol, hydrocortisone, benadryl, cetaphil, cocoa butter, etc., both before and after treatment. Nothing. Still itches and would redden/rash if I could stand to leave the patches on long enough. What should I do?

Hope you are keeping safe. I understand your concern, but was it on your own that you increased the dose to 9 mg and layer 12 mg or under your doctor's advice? 6 mg is a safe and well tolerated dose of Emsam and side effects like hives and rashes are common with higher doses. Symptomatic treatment for the rash is possible but it adviced to titrate the dose as soon as possible to tolerable levels after consulting your doctor as you can have serious side effects like breathlessness, shivering , sweating with high doses. You can apply Dermadew Caloe (Calamine + aloe vera) / Calosoft Lotion morning and evening for temporary relief. Kindly confirm with your doctor about the dosage and take a tolerable dose.
