Hello doctor,

I have been going through a rash with leathery skin on my genitals. It is dark when tight, but it is more of a pink color when loose. It feels prickly and itchy sometimes, with a burning sensation. I want to know if it is scrotal dermatitis or something else. Currently, I am taking Clobetasol Propionate Cream 0.05, Nystatin, and Triamcinolone. What to do to get rid of this, which has been occurring for the past five months?

Do you have any history of atopy or allergies? Did you use any harsh soap? Do you have any history of using scented moisturizer or lotion use? Did you scratch or rub the lesions excessively? Hi doctor,

Thank you for your reply. There is no allergies that I know of, and I do not use lotion or anything down there. I have been using white soap or Dr. Bronner's soap lately. Since I have been using the creams, it does not itch like that, but it still might have a mild burning feeling or feel prickly when I touch it. It used to itch really bad at night to the point where I had to scratch, but the cream solved that problem. I have never had a fungal infection before, and it does not feel dry to me, or it does not look dry either. Did you get a chance to look at the picture?

It appears to be Dermatitis due to mite allergies or genital scabies. Use Fucibet (Betamethasone and Fusidic Acid) cream at night for seven days and Permite BB (Elestab 0.1%) bar soap for bath for one month. Clean genita or affected area with Cetaphil cleanser or Episoft cleanser daily. Use 5% Permethrin cream on day one and day seven at night (contact period 8 to 12 hours) apply the cream on genitals. If there is burning, you can apply Lactocalamine lotion to cool and soothe the lesion. To moisturize the scrotal skin, use Aveeno moisturizer after the lesion heals. Avoid high potency steroids like Clobetasol. Wear loose, comfortable thin cotton inners. Maintain proper genital hygiene. Avoid using harsh soaps. Avoid scratching and rubbing of the lesions. Thank you for your reply. Is there any way you can prescribe these things to me because I cannot go and buy these things over-the-counter? And is there any way it could be anything else other than that? Where do mites come from? Did you get the pictures, or should I send another?

Night aggravation of itching is typical of Scabies. (It is a diagnostic sign of Scabies). I saw the picture (attachment removed to protect patient identity); there is only inflammation present. It could be due to the potent steroid use also (irritant contact dermatitis). Avoid possible irritants. So apply Fucibet cream at night for 7 days. Apply 5 percent Permethrin cream on day one and day seven (one week after) at night. Leave overnight for 8 to 12 hours and then wash off the next morning. Prescription cannot be given to overseas countries as per icliniq rules. So I cannot provide it. You can get it by showing this conversation or OTC (over-the-counter). Regards. Do you have any history of Herpes?Do you have a fever? Do you have any tingling sensation? Have you been tested for HSV (Herpes simplex virus) 2 or genital herpes? Because burning lesions or painful lesions are the symptoms of Herpes. Do you have a history of a recurrent episode? Do you have a history of swelling or inflammation of the glans penis? Do you have a history of recurrent Balanitis? Just get HSV 2 and IgM (Immunoglobulin M) test done to rule out genital Herpes.
