Hi doctor,

I am a 44-year-old male, and I have an itchy and reddish scrotum. Nine years ago, I had a fungal infection, and after treatment, it subsided. Since then, I have had few minor breakouts of fungal infection now and then, which were treated with Fluconazole and Candid cream. Four years ago, I suffered from scrotal dermatitis, and now for the last three years, I am having occasional mild itching, which increases at night. After having sex with my wife, it always causes infection to her the next day with burning sensation and itching in her vagina. I want to get rid of it entirely as my sex life is grieving. Please help.

The photograph attached (attachment removed to protect the patient's identity) is scrotal dermatitis. It is a feature of lichen simplex chronicus which means your itching aggravates the condition, and once you stop itching, the condition will also subside. I would suggest Mometasone cream twice daily over the affected area. After relief, keep applying moisturizer to prevent yourself from itching and tablet Xyzal M (Levocetirizine 5 mg and Montelukast 10 mg) to prevent itching. Scrotal dermatitis cannot cause itching in the vagina. Itching in the vagina of your wife could be due to candidal balanitis, which is causing candidal vaginitis, and you both are transmitting it to each other. It is a prevalent infection of the female genital tract. If you both have recurrent candidal infection attacks, then I suggest you both long-term tablet Fluconazole 150 mg once weekly for six months. Also, increase your frequency of sex because people having less sex complain of more burning. I hope it helps. If you have any other questions, please do not hesitate to contact me. Regards.
