Hello doctor,

My glans penis is dry and flaky, but not painful or itchy. Will it heal on its own? And only slight itch if there is an erection. I notice this started when I started brisk walking; I am not sure if it is related.

Apply Fucidin (Fusidic acid) cream twice daily (in the morning and at night) for seven days. Take Tablet Emanzen D (Serratiopeptidase and Diclofenac Sodium) twice daily for three days (if painful). Do you have any history of injury? Did you have a zip fastener injury? Do you have any history of atopy or allergies? Do you have any history of rough handling? Do you scratch excessively or rub the lesion? Did you use any harsh soap? Do reply.

A long time ago, I had a zip fastener injury. I have the itch at the back of my head, which comes and goes, such as itch and flaking, unsure if they are related. But the issue on my genital is not itchy, painful, and unusual as I have never had something like this before. I did not scratch or rub the lesion. I have been using the same soap for many years.

Use a non-soap cleanser like Episoft (Propylene glycol, D- Panthenol, Caprylic Triglyceride, and Glycerine) cleanser or no soap cleanser to clean genitals. Avoid the use of scented or harsh soap use on genitals. If there is irritation, take tablet Levocetirizine 5 mg at night for seven days. Please avoid scratching or rubbing lesions as it may cause secondary bacterial infection. Do you have any itching at the back of the head of the penis? Are the scalp flaky? Do you have any severe dandruff or seborrheic dermatitis? Do you have any family history of psoriasis? Are you diabetic? Attach the picture of the lesions to confirm the lesion.

Thank you for your reply, I have just had a health check-up recently, and I am not diabetic. Kindly refer to images. Sorry I mean, I have been having separate itching at the back of my head (hair) above the neck and also in my penis. The itching with flake mostly when I eat prawns. The place where peeling felt is like thick and dry skin.

It is eczematous dermatitis with scrotal dermatitis. Eczematous dermatitis is a kind of allergy in which there are dryness, itching, and pink rash. If a scrotal issue? There appears a pink rash. Avoid the use of coconut oil if using (it causes dryness). Maintain proper hygiene. Keep sugar levels under control. Take Tablet Levocetirizine 5 mg daily at night for ten days. Take tablet Allegra (Fexofenadine) 180 mg daily at night (if Levocetirizine makes you dizzy). Apply Hydrocortisone 1% cream in a very small quantity like a drop spread on itchy area daily at night for 15 days (use hydrocortisone not too much, and not more than two weeks). Use Fucidin cream twice daily (in morning and evening) for seven days. Take Tablet Zifi (Cefixime) 200 mg twice daily for five days (if oozing). Use good moisturizer at least three to four times daily. Aveeno (Dimethicone and Colloidal oatmeal), is really good moisturizer. Use Scalpe (Ketoconazole) shampoo twice weekly for six weeks (for scalp). If no improvement seen, please follow-up. Do you have any aggravating factor? Does there any seasonal variations? Is there any excessive use of lubricant or coconut oil?
