Hi doctor,

The skin around my eyes is getting scaly, darkened, and irritated from the last few days, especially near the canthus and outer canthus of my eye. Kindly suggest some treatment. I am suffering from PCOS for the last three years because I have a lot of hair fall and hair growth on my face and body despite laser treatment. My hairline is receding, and hair growth on the scalp is almost stopped. My hair texture has changed from being smooth to rough and dry. I am also suffering from weight gain, hirsutism, and acne on my back and chest. For this, I was taking Myoinositol, Progesterone, estradiol preparations, vitamin D sachets, and Metformin HCl. And recently, I have started ayurvedic medication like Myra and Neev for PCOS. I also did investigations like LH (luteinizing hormone), FSH(follicle-stimulating hormone), TSH (thyroid-stimulating hormone), testosterone, AMH(anti-mullerian hormone), OGTT(oral glucose tolerance test). Kindly suggest some medication.

Attach pictures to assess scalp hair density and treatment according to the grading of Alopecia. Do you have insufficient protein intake? Do you have a lack of proper nutrition? Do you experience stress? Do you have a thyroid disorder? Do you have anemia? Exercise regularly to keep hormonal levels stable. Attach pictures of lesions.

It is a female pattern hair loss with seborrheic dermatitis or excess dandruff. It is genetic and occurs due to hormonal changes. Also, dandruff can cause hair loss. Use Topical Minoxidil 5 % solution, apply 1 ml to the scalp (on hair loss or central parting area or hair thinning area) in the morning after washing the scalp and use 1 ml at night for three months, then 1 ml at night only for the next three months. Take tablet Follihair (vitamin, minerals, amino acids, and biotin) daily after breakfast for one month. Use Salisia KT (Ketoconazole and Salicylic acid) alternate shampoo day initially, then twice weekly for two weeks. If this shampoo does not control dandruff, use topical steroid lotion Mometasone (Elocon lotion)to be applied once daily at night at scalp skin and eyebrows two weeks. Then you can continue Ketoconazole shampoo (Ketoscalp) shampoo twice weekly for six weeks. Increase protein intake in your diet like nuts, milk, almonds, pulses, lentils, eggs, and dates. Use Cetaphil moisturizing lotion twice daily for 20 days (on dry skin). Use a comb that has vast space between each tooth, and the tip should be blunt. Avoid tight ponytail. Always comb gently, do not rush through. Follow-up for any doubts.
