Hello doctor,

I suffer from female pattern baldness, and I have been using Regaine (Minoxidil 5%) for almost a year. It has worked perfectly well for me, and I am happy with the results. Even my sister has been using it for the same complaint. However, now there seems to be a complication. Both sides of my head (where I apply Regaine the most) have become severely irritated to the point where dandruff flakes have built up, and I am constantly scratching. It has gotten so bad that the baby hairs I spent so long growing have fallen off, and my scalp is still damaged. What should I do? I have tried to use anti-dandruff shampoo and oil my hair every day. However, the complaint persists.

You have seborrheic dermatitis, which is dandruff with inflammation, in other words. I suggest you use Salisia KT shampoo (Ketoconazole and Salicylic Acid). Ketoconazole is antifungal, and Salicylic acid removes the scales. Use this shampoo thrice a week for two weeks, after two weeks, use it once a week for maintenance. Rest of the days, you can use any suitable shampoo. Do not use oil for the scalp, as it further aggravates dandruff. Seborrheic dermatitis is due to excessive sebum (oil) production from the sebaceous glands of scalp skin. If you put more oil, it will worsen the condition. However, you can use oil for hair tips to make them soft but not to the scalp.

Let me also list some hair care tips here:

  • Always use lukewarm water for washing hair, and never use hot water.
  • Do not comb when hair is wet.
  • Do not use a hair dryer; allow the hair to dry naturally after the bath.
  • Avoid hot iron straightener, as it breaks the protein bond in hair, making it brittle.
  • Hair is made up of a protein called keratin. So you need to take more protein daily, like eggs, fish, curd, milk, lentils, and chicken.
  • Do regular exercise for 40 minutes a day, six days a week, to keep the hormonal balance intact.
  • Use a comb that has wide spaces between each tooth, and the tip of the tooth should be blunt. Comb gently.
  • Avoid smoking. Nicotine decreases blood supply to the hair roots, affecting hair growth.
  • Avoid wearing a tight pony, as it would cause hair traction, eventually leading to hair fall from the frontal area. Instead, keep the hair loosely tied.
