Hello doctor,

My wife aged 43 years had fits (seizures) five years back. At that time, the blood reports, magnetic resonance imaging (MRI), and electroencephalogram (EEG) were normal and the doctor advised Epilex Chrono 300 mg twice a day. She used it for three years and then there was no episode of seizures during the last three years. So, on doctor's advice we stopped the medicine. Since then, for two years, there was no episode of seizures. Suddenly, two days back, she had fits around midnight. She was admitted to a hospital for a day, was treated and discharged. Now she was advised tab Celetoin 100 mg thrice a day. The computed tomography (CT) scan and chronic back pain (CBP) are normal as per the doctor. Is this medicine alright and how long we have to use it? Do we need any further investigations? Kindly advise. The doctor who treated her is a general physician.

Welcome to icliniq.com. She had a recurrence of seizures after stopping the medications. Usually, it is continued for five years after the second episode. If the previous magnetic resonance imaging (MRI) was normal and if it was a 3T MRI, then there is no need for any further investigations. If it was a 1.5T (Tesla) MRI then its better to get a 3T MRI done and look for the smallest of the abnormalities that may lead to seizures. I suggest she continue tablet Celetoin 100 mg thrice a day if she weighs more than 60 kg since the maximum dose is 5 mg/kg. If not, then reduce the dose according to the weight and continue that. Consult your specialist doctor, discuss with him or her and start taking the medicines with their consent. Take tablet Folic acid 5 mg in the afternoon every day.

My doubt is, earlier she had used Epilex Chrono and now Celetoin is given. Is that alright? Also, she is suffering from a severe pain in the right arm after the seizures. She is not able to move or lift the hand. Please advise.

Welcome back to icliniq.com. Both the drugs are almost equally efficacious with respect to seizure control. If Celetoin (Phenytoin) was given already, we cannot immediately switch over to another antiepileptic drug. Seizures at this age can cause fractures. Make sure that an orthopedician sees her. Get an X-ray of right arm and forearm.
