Q. How to deal severe fatigue with mental illness?

Hello doctor,

Patient is 16 years old. She is currently suffering from frequent fatigue. Before six months, she was mentally disturbed for which medicines were prescribed by a doctor. She was on Anxipose, Riscure LS. She did not have her menstrual cycle for quite a long period. And she usually vomits after eating.

The symptoms of fatigue could be a part of depression. I would need more information about what mental illness she had. If it is depression or anxiety, then she will need other medicines that what has been given. The medicines that have been currently prescribed are second-generation antipsychotics. One side effect of second-generation antipsychotics is increased levels of prolactin. This causes the menstrual cycle to stop.

Please get a serum prolactin level test done. It will be raised, and that means that she will have to stop the Risperidone and Olanzapine. Ask the doctor to consider using Aripiprazole, which will reverse raised prolactin and help with her behavioral symptoms.

In patients of her age group, fainting and unconsciousness happen due to two reasons.

1. Epilepsy, seizures, and fits - this is characterized by shaking movements of the body, froth coming out of the mouth, tongue bites, etc. This needs treatment with anticonvulsants or antiepileptic medicines.

2. Conversion disorder due to anxiety and stress is also very common. This needs treatment with antidepressants.

Either way, she has not been given treatment for these two situations.
