Patient's Query

Hello doctor,

I have the below symptoms:

  1. My entire body itched for the last five days. First three days, there was more itching, then on the fourth day, it reduced little, reduced more on the fifth day. Still mild itchy is there but no rashes.
  2. I feel like my anus has been burning and sometimes itchy for the last three days.
  3. Yesterday I found a small sore in my mouth, but today it is gone, and my lips are looking very dry.
  4. Today I feel colder even when I keep the air conditioning normal. I noticed some sores in my penis foreskin.
  5. Last month my weight was 78 kgs and yesterday it was 75.9 kgs. But I did not do any hard exercise. I go daily walking only.

Please clarify my symptoms.

Hello,

Welcome to icliniq.com.

I have read your query, and I am sorry for such discomfort. All these symptoms are tough to put under one diagnosis. So I need more detail about it. However, I will try to explain each symptom and find any common thread between them. Generalized body itching for five days and with weight loss can be significant. So here I am asking a few relevant questions about it.

  1. Is there any loss of appetite or yellowish eye, or any change in urine color?
  2. Is there any unusual tiredness or weakness? Any stress lately?
  3. Do you take any medicine for chronic illness?
  4. Do you have any recent sexual exposure history?

Other symptoms like feeling more cold and dry mouth etc., are one-day history and should be watched out for whether such symptoms are persistent or clear away within a few days. Please reply to the above questions and add any other medical history details to guide you more precisely.

Thanks.

Patient's Query

Hello doctor,

Thank you for the reply.

My eyes and urine are normal. There is no tiredness, and I am not under any medication. I had sex with someone 40 days ago with a condom, but she did oral sex without a condom. Yesterday, during the daytime, I felt colder, and during the night, I felt hot but had no sweat. Yesterday night I took a worm tablet.

Hello,

Welcome back to icliniq.com.

Now I have to shift my questionnaire regarding STI (sexually transmitted infection) based on the history you have provided.

  1. Do you know about the STI status of the female with whom you had sexual intercourse?
  2. Were there any urethral symptoms like pain or burning micturition?
  3. Do you have any discharge or fever?
  4. Is there any soreness or blisters in or around the penis?

Reply to the above questions as well to guide you further.

Thanks.

Patient's Query

Hello doctor,

Thank you for the reply.

I do not know her STI status. She is a sex worker, and one time the condom slipped off. That time my penis half part directly contacted her vagina three times. Then she adjusted my condom. Finally, she removed the condom with her hand, and she used the same hand to clean my penis foreskin with tissue. That time I did not notice any sore in my foreskin. Also, I am not aware whether the condom broke or not.

Further, I had minor skin damage on my finger. When my condom slipped off, I tried to adjust the condom. Maybe her vaginal fluid has contacted my damaged finger skin. The incident happened 40 days ago.

Now I do not have burning or pain in the urethral. There is no fever. Yesterday I saw two sores in my penis foreskin. Before that, there were two sores in the mouth, but they healed. I have been stressed for the last ten days. When I search for my symptoms on the internet, I fear that I may be infected with HIV. I am very nervous even if I notice any slight change in my body. Still, a mild itch is there. First two days, I felt itchier but had no rashes. I want to mention that I got itchiness in my legs last year with rashes. After I took the tablet, it healed. But this year, I did not take treatment for itching. I could not be normal. Please provide counseling. Thank you.

Hello,

Welcome back to icliniq.com.

I understand your concern.

At present, you have mentioned there are no STI (sexually transmitted infection) symptoms like urethral discharge, burning, painful micturition, or any blisters on or around the foreskin. But I cannot rule out STI based on this or vice versa, as there is a history of unprotected sexual intercourse.

Do not stress yourself too much. I suggest you take an STI panel blood test from any reputed laboratory in your area to rule out any STI to have peace of mind.

I suggest you take Levocetirizine for itching- one tablet before bed for five days.

Thanks.

Patient's Query

Hello doctor,

Thank you for your reply. I will go for a test soon. I want to ask you about the cold feeling during the daytime, especially on the feet when I am in air conditioning. But at night time I feel hot in the same air conditioning. Also, my lips are getting dry continuously. Can you clarify this, please? I would also like to inform you that currently, I am in a hot country where it is peak summer now. The temperature is above 43 degrees Celsius.

Hello,

Welcome back to icliniq.com.

I understand your concern.

This diurnal variation of temperature sensation is difficult for me to explain at this point in time. Let me know if it persists for some more time or if any other symptoms appear along the way with it.

Do take the recommended test, and you can consult me later with reports to guide you accordingly.

Take care.

Thanks.

Same symptoms don't mean you have the same problem. Consult a doctor now!
