Hi doctor,

I took Finasteride when I was 27 years old. It shattered me as a man. It has been four years, and now I am 31 years old. Things have not gotten better. I have all the symptoms of low testosterone. I feel I have lost my height, and my feet have shrunk. My penis and testes have gotten smaller, and my sex drive, energy, and memory have gone low. I have suicidal thoughts because of this. Please help. I do not think I can go on much longer. How should I treat this?

First of all, I like to assure you that there are multiple treatment options available for Finasteride-induced sexual dysfunction, and there is a little to worry about. Before I proceed to recommend any treatment, I request you to provide the following details. What do you mean by shattered me as a man? Do you mean the problem with erection, ejaculation, desire, or a combination of them? How did it start? Whether it was of sudden onset or gradual onset? How it progressed over the last four years? Whether the problem remains the same or worsened or improved to some extent? When you had the last satisfying and successful intercourse? What number of sexual encounters were satisfactory in the last three months? Do you get early morning erections? If yes, how frequent and intense are they? If no, how long are you not getting early morning erections? Do you have any medical or psychiatric illness (if yes, details of each)? Are you taking any other medication? I am glad to hear there are treatment options available for me. I need it. Yes, a combination of both. Weak erection. Arousal is difficult, and desire too has gone down. Also, the pleasure of orgasm is not as intense. Ejaculate volume is even less. My penis and testicles are not as big as before, as well as fewer morning erections. I do not feel like the man I was before. It was a sudden onset. I took the Finasteride, and within ten days, I had these symptoms. I never had any problems before my whole life. For four years, I have been carrying this secret and stress in my life. The symptoms have improved but not like before. I feel maybe I am only at 40% like before. I had intercourse this weekend. I can manage one sometimes two rounds. But sensation is less. Erection is not as hard, and I do not feel as horny or satisfied as before. Like my body does not feel as hot as before. Sensation in the penis and orgasm is not powerful or strong. All episodes have been satisfactory but not like before. Maybe I have sex two or three times a week. So 10 to 15 times a month. Morning erections are weak and do not last long. Not as frequent and powerful. No medical or psychiatric illnesses. I am not taking any other medication besides a multivitamin and omega-3 fatty acids. I have attached the new blood report. I also feel very tired, not enough energy, I think my memory is not as strong as before, and I am gaining weight.

First of all, I like to inform you that the current problem can be attributed to two factors. Prescription of Finasteride and its action on testosterone metabolism pathway. Stress of losing manhood and absence of appropriate help despite the passage of four years (you can call it chronic stress). The condition can collectively be called Post Finasteride Syndrome (PFS), a little known but hotly debated condition persisting after three months of Finasteride discontinuation. The available data is plenty, but contrasting information is also available. Regarding PFS treatment, I often recommend the use of Tadalafil 2.5 to 5 mg plus Dopamine agonists (Pramipexole 0.25 mg to 0.75 mg) daily. Although the response varies from person to person, most of my patients show good improvement. Add-on therapy with antioxidants often helps the remaining patients who show limited or no response using Tadalafil and Dopamine agonists. I hope this helps.
