Hi doctor,

I am a 31-year-old male. I got waxing done at the spa. In the end, the therapist gave me a hand job. Nothing else happened. She was fully clothed, and I did not touch her genitals. I did not notice any cuts or bleeding on her hands. She told me she did a massage on a client sometime before me and washed her hands. Am I at risk of any STD (Sexually Transmitted Disease) or HIV? I have done a 4th generation HIV test at 21, 39, and 47 days after this incident. All are non-reactive. I have attached the reports of the latest 4th generation HIV 1 and 2 test, which was taken after 47 days of possible exposure.

I saw your HIV test reports (attachments removed to protect the patient's identity). The chances of transmission of human immunodeficiency virus (HIV) by hand job are nil unless there are bleeding wounds on the hand. You have confirmed there was none in hand. Anyway, the test report has come negative. I suggest you do not need further HIV tests as there is no further risk of exposure.

Thank you very much for your reassurance. I have a couple of questions out of anxiety: When is the 4th generation HIV test conclusive for HIV 1 and 2? What if the massage therapist had semen from the previous customer hidden under her nails, or what if the previous customer's towel or bedsheet used for me was contaminated? As far as I remember, I did not notice any blood stains or semen on the surfaces. Please let me know about these and relieve me of anxiety. Also, let me know if further testing is not required.

The 4th generation antibody test is considered conclusive after three months of exposure; some consider it after one-month exposure. Intact skin is an effective barrier, so the risk of semen or blood on a towel or linen does not matter. I do not see any need for further human immunodeficiency virus (HIV) tests. If you are too anxious, I suggest you undergo an HIV antibodies test after three months of exposure.

Thank you very much for your response. But I did the 4th generation test three times at 21, 39, and 47 days from the incident, including the p24 antigen test. Also did only an antibody test on the 29th day. So there are four tests in total.

As I have suggested earlier, there was no need for HIV tests in your scenario. But you have got it done. So I suggest you not worry about HIV 3rd or 4th generation test. If you are too anxious, I suggest you undergo an HIV antibodies test after three months of exposure.

I got a fourth-generation HIV test done on the 91st day and an antibody-only test on the 93rd day. Both are non-reactive. Please let me know if I can consider the results conclusive.
