Patient's Query

Hello doctor,

I had protected sex two days after menstruation. Penetration was painful that day, and I had a lot of white discharge. I took an I-pill within a few hours and had another I-pill the next day. Since then, I have been having abdominal pain (dull pain) and white discharge some days. Ten days later, I had a slight bleeding that was watery initially and became sticky, slippery, and tiny clots. The bleeding was a little heavy the next day. What may be the cause? I was diagnosed with PCOS and am taking Metital. What are my chances of pregnancy? Are two pills within 72 hours harmful to my body? And why am I bleeding? Is this an effect of the pill, or is it implantation bleeding or something else? I am worried that I will get pregnant even after taking two i-pills, each containing 1500 mg of Levonorgestrel within 72 hours. When shall I expect my next period?

Thanks.

Hi,

Welcome to icliniq.com.

I understand your concern.

I have gone through your report (attachment removed to protect patient identity). As you are already aware, PCOS (polycystic ovary syndrome) leads to irregular periods. You have taken two emergency contraceptive pills within a short period, which is not recommended. These hormone-based pills can cause side effects like abdominal pain, nausea, light bleeding, or spotting seven days after intake. They also lead to delay or early onset of periods by up to seven days. You will not get pregnant since you have taken two pills, as these pills work in three different forms. Firstly, prevent the release of eggs. Secondly, avoid fertilization of eggs, and lastly, prevent egg implantation in the uterus. As far as the white discharge is concerned, please tell me if you are experiencing only white discharge or experiencing itching. Is the white discharge watery or like cottage cheese? Please answer the above questions to enable me to help you better. Nevertheless, it appears to be a vaginal infection, and for relief, apply Surfaz SN (Clotrimazole) cream locally. Use V-wash to clean the area. Keep the site dry. Wear loose cotton underwear. Do not do intense exercise.

I hope this information will help you.

Thanks.

Patient's Query

Hi doctor,

Thank you for the reply.

Now, it is just a watery discharge mixed with tiny clots. It is a light flow, and I have clots when urinating. I got very light blood in my pad yesterday, and today, I have a very light bleed until now. Am I experiencing withdrawal or implantation bleeding? And how long should withdrawal bleeding ideally last? I am worried that the double dose of I-pill will be very harmful if I am pregnant. Right now, I have minimal cramps.

Thanks.

Answered by Dr. Reetika

Hello,

Welcome back to icliniq.com.

I understand your concern.

You have taken a double dose of the emergency contraceptive pill. It will affect your cycles. That is what you see: mild spotting, brownish discharge, etc. You can wait until your expected date of the period. You can continue to have mild bleeding or spotting until your regular periods.

I hope this information will help you.

Thanks.

Patient's Query

Hi doctor,

Thank you for the reply.

Bleeding (spotting) is light today, nine days before my period. Is this withdrawal bleeding? It seems like it might stop today. Or can it be implantation bleeding? As you can understand, I am terrified. Also, does the double I-pill dose within two hours of intercourse and another amount within 54 hours somehow counteract each other? What are the chances of me not being pregnant?

Thank you.

Answered by Dr. Reetika

Hello,

Welcome back to icliniq.com.

I understand your concern.

It is unlikely that you are pregnant. Abnormal bleeding is due to the high dose of hormones. You have taken an I-pill within the period, so there is nothing else you can do about it. Just wait a week and then do a urine pregnancy test to alleviate your anxiety.

I hope this information will help you.

Thanks.

Patient's Query

Thank you doctor,

Can I be pregnant even after this bleeding? My bleeding has stopped now.

Answered by Dr. Reetika

Hello,

Welcome back to icliniq.com.

The success rate of emergency contraceptive pills is more than 90 percent if taken within 72 hours of unprotected intercourse. No one can tell you with 100% guarantee that you will not get pregnant. Yes, if you have taken the pill within the time frame, it is unlikely that you will be pregnant.

Patient's Query

Thank you doctor,

And what about the cramps I am facing. I am feeling a little nauseous and lighlt bleeding again. Are these cramps side effects of i-pill?

Answered by Dr. Reetika

Hello,

Welcome back to icliniq.com.

Yes, do not worry about the cramps. It is due to the side effects of i-pill.

Patient's Query

Thank you doctor,

When does implanation bleeding occur and how does it look?

Answered by Dr. Reetika

Hello,

Welcome back to icliniq.com.

Implantation bleeding is the light bleeding or spotting female experiences after two weeks (10-14 days) of conceiving. It is generally around the time of the period of the female. However, few ladies experience it, and few do not experience it. Some ladies misunderstood implantation bleeding as a period and vice versa. It is very light bleeding, or it will be just spotting of blood.

Patient's Query

Thank you doctor,

I had sex two days after my period stop, and I took two pills and started bleeding after ten days. What is the chance that this is implantation bleeding? I also have Metital twice a day. Can that counter effect i-pill?

Answered by Dr. Reetika

Hello,

Welcome back to icliniq.com.

Since you had protected intercourse and more to that, you had two i-pills. There is no chance for you to conceive in such a short time and hence no chance of implantation bleeding.

Patient's Query

Thank you doctor,

Does having two i-pills in a short time counter effect each other? I read that withdrawal bleeding is like a period only, but I had so light bleeding and only three days. Is that a cause for concern?

Answered by Dr. Reetika

Hello,

Welcome back to icliniq.com.

Your first query is not correct as per my knowledge. The second query is valid. For some ladies, it is light bleeding. Every human body is different and behaves differently. Some do not experience any withdrawal bleeding.

Patient's Query

Mam, can you tell me how ipill works, what does withdrawal bleeding mean, shall i consider this withdrawal bleeding or just spotting

Please wait till your expected date of periods to be sure. Ipill works by three mechanism: inhibits ovulation, prevent fertilization and hamper implantation.

Patient's Query

Hi doctor,

Thanks for the reply.

My period has still not come. Is that a sign of pregnancy, or what might be the case?

Thanks.

Answered by Dr. Reetika

Hi,

Welcome back to icliniq.com.

I understand your concern.

You can wait for a week. But if you take so much stress, it will further delay your periods. If you want, you can do a urine pregnancy test.

I hope this information will help you.

Thanks.
