Did you notice any change in the appearance of the lesion? Any family history of similar lesions? The current medications, as you have mentioned in the history, are for the treatment of tinea infections which are fungal infections. In the case of tinea, the medicines need to be used for at least eight weeks to notice changes and reduce symptoms and intensity of lesions. Keto 4S cream, as mentioned, when used on tinea infections, would not show changes as it is a combination cream with ingredients contradicting the treatment of tinea, which may result in a flare-up of the current condition. Therefore, I would suggest you to stop the medications for three days and undergo the following before starting the treatment, depending on the report. KOH (potassium hydroxide) skin scraping test, where the skin scalings are scraped gently and tested to see the presence of fungal elements.
