Patient's Query

Hi doctor,

My daughter has a skin problem in the torso area that is spreading to her arms, legs, chin, and cheeks but it is not causing any itching. She is allergic to milk but the allergist said to give her things baked in milk like bread or mac and cheese, etc. After using the steroid cream for several days, it got lighter but returned immediately. She had the same issue previously that went on for more than nine months. Currently, we are not using any medication but in the past, she was prescribed a steroid cream. No laboratory tests are done so far.

Hello,

Welcome to icliniq.com.

I read your query and can understand your concern.

I reviewed the attached images (attachment removed to protect the patient's identity), and I need to ask you a few questions.

1) What is the age of the child?

2) Is there any history of dandruff, dry skin, allergic rhinitis, or asthma in the child or family members?

3) Which steroid cream have you used in the past?

4) Is there any history of eczema in the family?

Kindly revert with the answers.

Patient's Query

Hi doctor,

Thank you for your reply.

This year, she turned two. There is no history of dry skin, dandruff, eczema, rhinitis, or asthma in the family. Mometasone furoate cream was previously administered to her. Most of the time, it does not affect her, but a few times after applying it, she will grab it, causing her immediate discomfort.

Hi,

Welcome back to icliniq.com.

I read your query and can understand your concern.

According to your complaints and uploaded pictures (attachment removed to protect the patient's identity), it seems to be an eczematous rash. Itching is a subjective symptom and can vary between individuals. I suggest you apply a topical steroid like Cortisone cream twice daily along with the frequent and liberal application of a moisturizing lotion that contains white soft paraffin 3 to 4 times daily. Also instead of soap, opt for soap-free cleanser and use lukewarm water for bathing instead of hot water. Make her stay away from dust and pets, and do not make her wear woolen and synthetic clothes that might come in direct contact with her skin's surface. Once her condition improves taper down the steroids. Do not stop them immediately, otherwise, eczema will recur.

Hope this helps.

Thanks and take care.
