Hi doctor,

I have never had acne or any skin problems in the past. The breakout comes and goes, off and on lower abdominal pain with gas but constant tenderness in the lower abdomen. I seem to always be cold lately and extremely tired all time, no matter how much I sleep. I have always struggled with fatigue, only just worse now never had bowel movements regularly, but it never seemed to bother me in the past. I am on the following medications, Adderall, Prozac, Propranolol, Buspirone, and a daily vitamin.

Now I have a rash on my forehead, cheeks, nose, and chin with pus-filled bumps. Lower abdomen tenderness breaks out of these awful pimples that turn into burning sores all over my chest, shoulders, and back. Yellow colored skin underneath my eyes between my eyes and cheek rash. Extremely fatigued and weak. I have an appointment this week, but hoping to get an idea of what could be going on.

I have gone through your description of symptoms and understand your concern.

Had a look at your images also (the attachments are removed to protect the patient's identity).

The possibilities which I could think of include the following:

1. Streptococcal skin infection.

2. Autoimmune disease, associated rash, systemic lupus erythematosis, or sarcoidosis.

3. Drug-induced.

Among the drugs you mentioned, Adderall and Buspirone can cause skin rashes as side effects.

A close watch on your skin lesions can give more information regarding the cause.

You can discuss the same, which I mentioned with your doctor on Monday.

Antibiotics and steroids might help with your condition, but I am not prescribing any since they may alter your skin condition and lead to confusion in diagnosis when you visit the doctor.

I suggest certain blood investigations, which you can do before your doctor visit, and consult with your doctor with the reports that may help them for diagnosis.

I suggest you do the following investigations:

  • ANA (antinuclear antibody).
  • ENA (extractable nuclear antigen) profile.
  • CBC (complete blood count).
