Hello doctor,

I had consulted for skin rashes with a doctor. The discussion is as follows. Me: I have been suffering from rashes for the last five days. The itching is not so worrying but the rashes developing gradually. For this, I have taken antiallergic drug Montek and applying Aloe vera gel in the affected area but the thing is not improving what should I do? Doctor: It looks lie Tinea incognito (fungal infection with additional allergic eczematization). You can start Onabet B (Sertaconazole, Beclomethasone) cream at morning for seven days, Tacroz 0.03% ointment at night for 15 days, Eberconazole 1% (Ebernet) cream at afternoon for two weeks, and tablet Levocetirizine 5 mg one tablet at night for 15 days. Follow up after 10 days. Now though the situation has improved, the rashes are occurring on the other side of the hand along with itch. I have applied all the medicines prescribed by the doctor. Please suggest what to do? Along with this, I am sending previous and current photographs of the affected areas. Too much itching persists. Please help.

Yes, I have seen the pictures now (attachment removed to protect patient identity). Have you had any recent dust, food, or metal allergy? Do you have a history of wheezing or asthma? Any history of contact with plants, detergents, or anything history relating to the onset of these lesions? When you itch, does watery fluid ooze out from the lesion? From the photos, it looks more like dermatitis or eczema to me, which is an allergy to something maybe with a co-existing fungal infection. Please answer my queries.

I feel some allergy if I eat brinjal and egg. But since the last 10 days, I am not taking any. No history of wheezing or asthma. No watery fluid ooze out from lesion. Itching mostly occurs during night time.

I used to take Eltroxin 100mg and Repace H tablets. I am wearing this imitation bangle always. During the summer season it occurs most.

You can use Desonide lotion for application in the night only on the lesions for seven days, Dermadew Caloe lotion (Aloe vera+ Calamine) lotion in the morning and afternoon, tablet Teczine M (Montelukast 10 mg + Levocetirizine 5 mg) once daily at night, Ketoconazole soap for wash, Fudic (Fusidic acid) cream for open wounds at morning and night. Review after one week. You can stop on the other medications since you have used it for 10 days. Avoid suspicious triggering factors and avoid wearing any bangles for the time being. Avoid imitation jewelry as it is possible to develop a delayed allergic reaction to it. Avoid prolonged exposure to water. Avoid exposure to detergents and soap cleansers. Avoid meat and poultry products, dairy products, fish, pineapple, citrus fruits, nuts and chocolate, eggs, soy and wheat items. If even after a week there is no improvement, then you may have to do certain blood tests. Do complete blood routine, ESR (erythrocyte sedimentation rate), IgE (immunoglobulin E) levels, dental follow up to rule out caries, urine routine. Any such allergy can be triggered easily by photosensitivity on sunlight. So please avoid getting out in the sun for few days.
