Hello doctor,

I am 29 years old, and my wife is 25 years old. We got married two years back. We have been trying to have intercourse since then but could not get it done. We both involve proper foreplay and have good sexual arousal. I experience good erection, and she also gets lubricated well inside. But, by the time I insert my penis into her vagina, we find it very hard to get the insertion done. She experiences pain, and I find it too tight to insert completely, making the penis slip away upwards or sideways. Eventually, I am losing erection by then and losing confidence to try again. Every time the same thing happens, and it has been bothering us much. We find the foreplay orgasm mutually normal, but when I try to insert my fingers, gradually increasing the number of fingers to make her feel better, by the time I try to insert two fingers, she is experiencing pain. I am expecting an expert's advice and a solution to our problem.

Is she getting her monthly periods properly? Does she undergo any surgery?

For the past three to four months, she has been getting her periods regularly without any delay. No surgeries are done. We tried using lubricants and, to an extent, aloe vera oil too. But it has not helped. It will be of great help if you can let us know the exercises which you had mentioned. Can I use the gel which you mentioned as a lubricant? Any specific directions like when and where to apply? Is there anything that I need to take care of or any medication to keep my erection? After our first try, every time, out of anxiety or tension, I am losing it that I might hurt her since she suffers from pain.

There is no problem with you both. The problem your wife is facing is just a psychological fear, and that condition is called vaginismus, i.e., voluntary vaginal tightening. She is afraid of the pain of intercourse. Just counsel her about this. Yes, you can use this Lignocaine lubrication to be used before intercourse. Apply Lignocaine gel to your penis before penetration in an erected state. It will be smooth and cause less pain. But initially, it will pain. Once she can take up and tolerate pain initially, everything will be fine. She has to do Kegel exercises regularly to strengthen her pelvic floor muscles. Now, to your erectile state. Even you can do pelvic exercises to prevent premature ejaculation. You both should start taking a multivitamin to strengthen your sperm and her ovum (egg). Take tablet Fertisure-M (for your husband) and tablet Fertisure-F (for your wife). Both tablets one in the nighttime after food. Ask your wife to take a tablet of Letrozole 5 mg (from the third day of the menstrual cycle to the seventh day - 5 tablets for five days). You both will receive good news soon. Be calm and relaxed. Avoid anxiety.

We tried it a few times later, but could not get it done. The same case follows, I lose erection halfway and still, the same problem of being unable to penetrate exists. I think she should tolerate some kind of pain initially or else it will be difficult for her to conceive. So just convince her. And for your erection you can you tablet. Take this tablet 3 hours before sexual activity and not intended for daily use and you can use only 3 tablets maximum in 10 days.
