Hi doctor,

I am a 22-year-old male, height is 5'10, and my weight is 72 kg. I had smegma when I was around 18 years old, and at that time, it went away naturally after like one month. I do not remember now. I started seeing smegma two weeks ago under my foreskin and a few red spots. I do not have any other health conditions or take any other treatments. I do not feel pain, but it tickles a little bit when the smegma is there. When I wash it, the smegma goes away, but the next day, if I do not wash it, it returns. Kindly help.

I can understand your concern. Smegma is a normal secretion in males; it will be excess if you sweat a lot. I suggest you wash it every day with just water during the bath. If you do not wash it every day it can accumulate and cause infection and a bad odor. Better assessment can be done if you share the picture of the red spot. If there was unprotected sex, we need to consider sexually transmitted infection. I also suggest you do the following tests:

1. TPHA (Treponema pallidum haemagglutination) test.

2. Human papillomavirus deoxyribonucleic acid detection by polymerase chain reaction (HPV DNA PCR) test.

3. Herpes simplex virus (HSV) 1 and 2 immunoglobulin G (IgG).

4. Herpes simplex virus (HSV) 1 and 2 immunoglobulin M (IgM) and serum.

5. Chlamydia trachomatis immunoglobulin G (IgG) test.

6. HIV (human immunodeficiency virus) 1 and 2 antibodies screening test.

7. Hepatitis B surface antigen (HbsAg) test.

If the test results are negative, it can be a simple bacterial infection. Therefore, I suggest you use ointment T-Bact (Mupirocin) for local application and tablet Doxy (Doxycycline) 100 mg twice daily for five days.

Suppose there is itching; we need to think of a fungal infection. In that case, I suggest you apply ointment Candid-B (Beclometasone and Clotrimazole) for local application and Tab Cetzin (Cetirizine) once at night.

I also suggest you wash your genitals daily, morning and night, using normal water and not heavy pH soap.
