Patient's Query

Hi doctor,

I had prolonged spotting for one month followed by bleeding for 2 to 3 weeks. I took Meprate twice a day for three days, followed by one tablet each day. Bleeding stopped, but while I was taking Meprate on eighth day I did heavy workout and spotting started again. What shall I do? When should I take next Meprate dose.

Meprate is not the treatment for bleeding. It is a Progesterone and the dose is fixed, you cannot reduce it on your own as you would start getting withdrawal bleed once dosage is reduced, which is what has happened in your case. Once you stop Meprate, you shall again get at the bleed in next 3 to 5 days and may be heavy, as it would be a withdrawal bleed post-Progesterone intake.

Firstly, you need to stop Meprate right away. The bleeding shall increase possibly in the next two days which would be the Progesterone withdrawal bleed as would be initiated by stopping Meprate now. If the bleeding is heavy or more than three pads per day then you can start antifibrinolytics like tablet Tranexamic acid 500 mg with Mefenamic acid 250 mg (combination) one tablet thrice a day for two days and then twice a day from the third day till bleeding stops completely. But, before starting it, I would like to ensure that you do not have any bleeding disorder, in such cases you should not be opting for it. Also, ensure adequate hydration, water intake as sometimes women complain of dizziness due to excessive bleeding.

You are most welcome. Let me know if the bleeding is controlled. I would like you to send me an ultrasound pelvis for evaluation once the bleeding stops. If I may ask what was the reason for starting Meprate cyclically for 21 days for three months by your doctor? What was the diagnosis offered?
