Hello doctor,

I am a 67-year-old female who is a stage 3 CKD patient with proteinuria. I am a rheumatoid arthritis patient for 30 years and am wheelchair-bound. I have had excessive itching and muscular pain for the last few days. My creatinine level is 1.8. Currently, I am taking Dytor, Ciplox, Oxemia 50 mg, and HCQS.

Thank you for your query.

I understand your concern regarding itching. Itching may happen due to various reasons in CKD (chronic kidney disease) patients like increased urea or high phosphate, but these are normal here. Secondly, a high amount of parathormone may cause this. I would advise you to get this test done if not done already. I suggest you answer a few questions mentioned below.

  1. Since when are you taking HCQS as this drug is known to cause skin problems and itching
  2. Are there any skin rashes?

You can take anti-allergic drugs like Levocetirizine or histocalamine lotion for itching. Get vitamin D3 and iPTH (intact parathyroid hormone) levels checked to evaluate for muscle pain. Your albumin level is low which may cause weakness and muscle aches. You need to increase protein in your diet to at least 0.6g/kg per day.

I will surely get the tests done. Is Tums antacid safe for CKD patients? I have read a lot of patients getting relief from itching after having it.

Yes, Tums can be taken by CKD (chronic kidney disease) patients, but I am not sure whether it can relieve the itching as it is not used in our daily practice.
