Hi doctor,

My father had a stroke, after which he is not interested in eating or drinking, lacks interest, and sleeps excessively in the daytime. His recovery seems extremely slow.

There can be multiple factors (single or in combination) for such symptoms after a stroke at this age:

1. Stroke itself may be a reason if it affects the brain's thalamus.

2. Post-stroke depression is a widespread phenomenon.

3. Underlying dementia, on which stroke causes a further decline in brain reserve.

4. Associated electrolyte or metabolic derangements.

5. Superadded infective process, particularly in the lung or urine.

Investigations to be done

The investigations to be done are serum ammonia, chest X-ray PA (posteroanterior) view, routine urine test, CBC (complete blood count), LFT (liver function test), and KFT (kidney function test).

Treatment plan

Keeping all mentioned possibilities, you can physically consult a neurologist or psychiatrist through chat message if it is not feasible to narrow down the possibilities.

Preventive measures

Try to keep the patient awake and involved in some household activities. Ask the patient to communicate with family members during the daytime. Avoid daytime somnolence. Exposure to sunlight daily in the daytime is necessary. Try to keep the patient moving during the daytime. Rule out secondary infectious or deranged metabolic parameters. Keep reasonable control over blood pressure and sugar levels.
