Hello doctor,

I have swollen feet which are getting worse day by day. My feet feel heavy. I do not eat food with salt and try to drink a lot of water. What could it be?

1. Have you done any recent blood tests like KFT (kidney function tests), or LFT (liver function tests)?

2. Are you taking any medication for heart disease?

3. Have you done a two-dimensional echocardiography test to know the ejection fraction of the heart?

4. Do you suffer from any breathing difficulty or chest pain?

5. How is your micturition?

I want to know your KFT with albumin level and heart function status to guide you on the cause of edema and medication. Kindly reply to the above questions to guide you on diuretic medications to reduce lower limb swelling.

Thanks for your reply.

Yes, the blood test was normal. I am not taking any medication for heart disease. I have not done a 2D echo test for the heart. I do not have any chest pain or difficulty breathing. The micturition is normal. Could you prescribe a medicine or ointment to reduce the swollen feet?

If KFT and serum sodium and potassium levels are normal, I suggest you take a tablet of Dytor (Torsemide) 5 milligrams once daily for ten days. It will help to reduce swelling by increasing urination. Maintain adequate hydration and avoid salt intake. Also, please attach a picture if possible to assess it.

There is pitting edema present on the left foot. The other foot is not visible. Is the other foot having similar edema?

I suggest you follow the instructions mentioned below.

1. Get KFT (kidney function test) and LFT tests (liver function test) done.

2. Do a two-dimensional echocardiography test to know the ejection fraction of the heart.

3. Do a thyroid profile test.

4. Take a tablet of Dytor (Torsemide) 5 milligrams once daily for ten days.

5. Maintain adequate hydration as there would be an increased amount of urination after taking the above tablet.

I forgot to tell you that a month ago my father had a urine problem. He used to feel like he wants to go to pee but the urine do not come out.

Usually, swelling in both feet means either there is a problem related to kidney or heart function. Another reason is low albumin level, and in that condition, there would be swelling of the entire body and not confined to the lower limb only. Along with the above tests, I would like you to do an ultrasound of the kidney, ureter, and bladder region and also to look for kidney and prostate size. Usually, increased prostate size cause the above symptoms. It is called benign prostate hypertrophy.
