Hello doctor,

I have anxiety and panic attacks from last six years. My doctor prescribed Nexito Forte 10 mg. My doctor tapered the tablet to Nexito Plus 5 mg three years back. My questions are: Is the relapse due to decreasing the dosage? Should I increase it again?

The dose of the particular drug depends upon many reasons. So, it is difficult to tell that this dose is effective or not for the particular person. Now coming back to your questions, first Nexito Plus (Escitalopram and Clonazepam) has only half of the dose compared to Nexito Forte (Escitalopram and Clonazepam). So, when tapering the dose, few persons may relapse. If you develop the symptoms of anxiety, you need to increase the dose again. And it is quite difficult to predict who is going to relapse. Secondly, as the Nexito Plus has a lower dose of the drug, it is quite prudent that it may not be effective in all persons. So, increasing the dose is a good option. Similarly, when one is tapering the dose, some symptoms may reoccur. Then one has to slow down the pace of tapering as to avoid the relapse. So, if you are stable on Nexito Plus, then no need to increase the dose, but if you have symptoms then you need to increase it. The dose is basically changed according to the response and side effects, one is tolerating. It is individually designed to meet the requirements of that particular person. Thanks and regards.
