Hi doctor,

I am 37 years old. I have mild pain in left testes and it hangs more than usual. This happens for the last four to five days. I feel discomfort during walking and mild pain in the lower abdomen just above the left testicle. Regarding my current medical complaint, this is the first time I have experienced such pain. I noticed it when I attempted to touch my toe by bending my upper body. Please help.

Hope you are doing well. Coming back to your query, I think you may have an infection in your left testis, a condition known as epididymo-orchitis. First, you need to provide scrotal support (a langot or relatively small V-shaped underwear) to support your testes from hanging. Second, start the medication as prescribed (see attachment). Are you diabetic? Do you have any urinary complaints (night frequency, burning micturition, poor stream)? Any previous history of accidents, trauma, or urinary catheterization? Are you on any medications? Do you smoke or consume alcohol? If you can click a photo of both testes, it will be great. Lastly, the ultrasound you uploaded has photos (attachment removed to protect patient identity). If you can upload a picture of the report, it will be better.

Thank you for the reply.

I am not diabetic. No urinary complaints. No burning during urination. No, I am not on any medications, and no trauma. I usually take a 2.02 fluid ounce two-pack of alcohol once a week.

Yes, from the pictures (attachment removed to protect patient identity), it seems to be epididymo-orchitis only. You have to continue the same medications as I have prescribed. You can contact me again after five days of medication with scrotal support.

Hello doctor,

Thank you again for your time; I greatly appreciate it.

I contacted you two months back regarding swelling of the left testes and mild pain. After taking the medication (five days course) and using scrotal support (langot), I felt better for two to three days, but now I am experiencing these symptoms again. Please refer to the attached testes photos. Please give your opinion.

I can understand your concern.

If you really feel that symptoms have reappeared, then I will suggest you get an ultrasound of the scrotum. Clinically, I feel it is epididymo-orchitis only. There is no change in treatment, and you have to continue the same. How ultrasound will help: firstly, it will confirm the diagnosis, and then it will rule out any testicular abscess (though unlikely). In that case, you might need some kind of intervention. The swelling will go gradually, but the pain should have subsided by now (if proper scrotal support was given). I know it is a little difficult to get an ultrasound of the scrotum done, but if the symptoms are not improving, it is better to have one.

I have reviewed the attached reports (attachment removed to protect the patient’s identity). It is epididymo-orchitis only on the left side, which is causing the problem. You have to continue the same treatment (sometimes it has to be taken for 10 to 14 days). Bilateral varicocele is also there, which causes a dragging sensation or heaviness most of the time. I do not think you require surgical intervention for varicocele. Scrotal support will help to get rid of that pain too. Hydrocele is reactive and will settle once orchitis settles. I suggest you to add tablet Chymoral forte (a combination of Trypsin and Chymotrypsin) twice daily for ten days (it is an anti-inflammatory, so it helps in decreasing inflammation faster). Consult your specialist doctor, discuss with them, and take the medicines with their consent.
