Hi doctor,

I have had thickening of my left spermatic cord and pain in my left testicle for over one and a half years. It all started after unprotected anal and oral sex. I have got MRI and nerve block done. But the pain still persists. I am taking a pain management course, Pregabid NT 75, Pregabid 50, and Nortimer 25 mg. Previously I was taking Gabapentin, Dulane, and Neurokind. I have also got lab tests such as CBC, urine routine, urine culture, semen culture, C-reactive protein, folate, ESR Westergen EDTA done. Kindly help me.

This is a previous infection caused by unprotected sex and leads to fibrosis around the cord; hence there is pain, step ladder treatment which is analgesia tablets local injections and that you already tried. Now next is going to be surgery which is called denervation of the cord, which is indicated in your condition. It is to open both inguinal areas and remove the nerve which is causing pain. But there are few cases after surgery that will continue to have pain. Unfortunately, the fibrosis is irreversible, but the good point is pudendal nerve intact. Hopefully, with surgery, things are going to be fine. Surgery can be done by a urologist who usually performs this procedure, and he will explain the risks and possible complications from it.
