Patient's Query

Hi doctor,

I am 44 years old. I am suffering from testicle pain, penis discharge, and frequent urination after having a sexual relationship. I have taken Zithromax and Ceftriaxone 1000 milligrams three times.

Please help me.

Thank you.

Hi,

Welcome to icliniq.com.

These symptoms show no response to antibiotics. You have a fungal Infection of the urinary system. You need to take oral anti-fungal capsules like Fluconazole 150 milligrams daily for 7 days.

Thank you.

Patient's Query

Hi doctor,

Thank you for your answer.

I have done tests for the urinary infection and they show nothing in tests, but in fact, I have burning and frequent urination. I have done all the tests as you have seen. My girlfriend also had all the tests but she has nothing. She has done STD tests, and they were clear. The only antibiotic that has affected me is Cefixime but after 7 days the pain in my testicle and a few yellow secretion shows. I will take Fluconazole 150 milligrams for 7 days and reply to you back.

Thank you.

Hi,

Welcome back to icliniq.com.

This seems fungal infection which will not be treated with anti-bacterial medication. And the tests you shared (attachment removed for patient identity protection) do not have urine testing or culture sensitivity tests. Special kits are used to test for fungal infection.

Do let me know after taking Fluconazole for a week.

Thank you.

Patient's Query

Hi doctor,

I am on day six of taking Fluconazole. The last three months (if I forgot to mention) have changed the odor of the sweating, I have a bad smell in sweating as well. I never had it before. On the first day of taking Fluconazole, the secretions had disappeared completely, and the pain has reduced to 50 % but still not completely. Today I noticed I had secretions from my penis again, not during the day but at night only. I can distinguish them from my pants only, there is a small mark in white color in pants. Does my partner need to be treated with Fluconazole as well? I mean do I need to have a protected relationship with my partner?

Thank you.

Hi,

Welcome back to icliniq.com.

Ideally, she should take Fluconazole for a week as well. Protected intercourse is a must for the next two weeks at least.

Your condition seems to be improving. Please continue Fluconazole for the next four days, for a total of 10 days.

Thank you.

Patient's Query

Hi doctor,

Thank you for your answer.

Is it normal to have a bad smell of my sweat from fungus?

I never had it before.

Thank you.

Hi,

Welcome back to icliniq.com.

Yes, it may be a cause of the bad odor.

Thank you.

Patient's Query

Hi doctor,

It is almost four days since I finished Fluconazole. I have had no more secretions until now, but the pain in my testicles is still present in the morning. I do not have pain but it amplifies until the evening when the pain becomes much stronger. If I seat, I have more pain. If I walk, the pain is much smaller.

Can you suggest to me how to proceed?

If it is still fungus, do I have another option of medicine?

Is the pain normal if I have fungus?

I have made an echo of my testicles two months before when I had this pain, and the doctor told me I have nothing from the echo (no varicocele, no epidermitis).

Please suggest.

Thank you.

Hi,

Welcome back to icliniq.com.

Good to hear that you no more have the secretions. The pain you mentioned was due to severe inflammation in the testes you experienced due to a fungal infection. It will take a week or so to go away. Try to wear loosely fitted undergarments and avoid long-standing hours. Avoid any sexual activity for a further week.

Take Sitz bath daily. The pain will settle soon.

Thank you.

Patient's Query

Hello doctor,

Generally, I have no secretions, but some days when I wake up I have a few secretions. During the day I have no more secretions. What bothers me is the testicular pain, it become more severe and sometimes I have some strong pains for some moments. I am following a diet also, presuming this can be candida, but with no effect. This is almost day 5 after I finished fluconazole, I had no sexual relationship. What do you recommend me, sometimes it seems that the pain continues to the legs for a moment below the testicles. The pain disturbs me a lot, especially when sitting and especially near evening.
